UPDATE as of 11/25/2012 at 12:07 PM EST: Please do not send error requests that are contain the following:

"If you help me, I will give you..." is NOT allowed!

"No skipping, Do not skip this error, accept or die, etc." are not allowed!

UPDATE as of 1/4/2013 at 4:07 PM EST: New rule for error requests: I am only accepting error requests that are very funny. If your error request does not make it into any of my videos, do not get mad because I will still have a bunch of errors from you.

UPDATE as of 7/9/2014 at 3:14 PM EST: Voting on error requests is no longer going to be supported as it has been causing too many issues. This is the new Error Request Form that was modeled after the new Sign Request Form.
